Client Anesthesia/Surgery Consent Form

Thank you for choosing Press Paws Surgery for your pet’s surgical needs. The following consent form must be submitted to Dr. Collins prior to the day of surgery. Your primary veterinarian will have their own consent form for you to fill out separate from the one provided by Press Paws. Failure to submit the necessary paperwork may result in cancellation of your pet’s surgery.
